Provider application
Tell us about you and your practice. We support solo providers, clinic groups, and multidisciplinary teams.
Licence & college verification
We verify your standing with your Ontario regulatory college (CCO, CMTO, CPO, CRPO) and any additional required credentials.
Discipline & location setup
Configure the disciplines you offer, virtual vs in-person care, and each clinic location you work from.
Master Provider Agreement review
Review the Master Provider Agreement with independent legal counsel and sign digitally.
Insurance / billing profile setup
Configure your service list, pricing, insurance receipt details, and connect your payout account (Stripe Connect).
Portal access
Full access to your Coalition provider portal — profile, referrals, compliance alerts, receipts, and payouts.
Go-live on BelongHealth.ca
You start receiving eligible referrals and appointment requests from the Belong Health network.
Documents you'll upload
Everything is encrypted at rest and only accessible to you and the compliance team reviewing your file.
- Government-issued photo ID
- Professional college registration certificate
- Proof of liability insurance (with expiry)
- Clinic / practice location address(es)
- HST / business number, if applicable
- Direct deposit / payout account details
- Service list and pricing
- Consent to verification and audit
